Reporting Student Concerns
McHenry County College is committed to maintaining the safety and well-being of the campus environment. Each of us, as students, faculty, and staff, share responsibility for ensuring that our campus is a safe and respectful place to learn and work. So, if you have experienced something that causes you concern, or simply doesn’t seem right, please report it.
There are different forms for each type of concern, but there is no “wrong” way to report it! An administrator will receive your report and, after discussing it with you, will ensure it is routed to the correct person or department for resolution.

Threatening or Concerning Behavior
Use this form to report threatening, or otherwise concerning, behavior that you have observed. Examples include (but are not limited to): direct or indirect threats to harm self or others, significant mental health concerns, or behaviors that cause concern for future violence.
Reports are received by the Behavioral Intervention Team, which will assess the report and determine the next steps for a response.
Note: This report should not be used for emergency situations. In a life-threatening emergency, call 911. You may also call campus police at 8733 from any campus phone, or (815) 455-8733 from a cell phone.

Disruptive Behavior
Use this form to report behavior that disrupts the campus learning environment and may be a violation of the Student Code of Conduct.
Reports are received by Sam Clifford, Coordinator of Student Advocacy. They will reach out to you to discuss the report and resolution options. You can also contact Sam at (815) 455-8794.

Sexual Harassment, Assault, or Sex- or Gender-Based Discrimination (Title IX)
Use this form to report sexual harassment, sexual assault, discrimination based on sex or gender, or other sexual misconduct.
Reports are received by Rachel Boldman, Director of Student Engagement and Support and Title IX Coordinator. She will reach out to you for more details and to discuss further options with you. You may also make a report by contacting Rachel at (815) 479-7572 or rboldman@mchenry.edu.

Bias or Discrimination
Use this form to report incidents of bias or discrimination that you have experienced or witnessed.

General Complaint or Grievance
Use this form to report incidents of unfair treatment or harm at MCC that may not fit into the above categories.

Use this form if you have general feedback, positive or negative, about your experience at the College.
