   • Topics in Early Childhood Education (ECE 290)
   • First Responder Emergency Aid (EMS 105)
   • Introduction to Horticulture (HRT 100)
   • Elementary Algebra (MAT 095) and Intermediate Algebra (MAT 099)
   • Principles of Marketing (MKT 110) Have the student fill in personal information
2. Have instructor sign agreement
3. Make three copies
   • The instructor keeps a copy for records
   • One copy is for the student
   • The instructor mails the original agreement and a copy to:
